340B Discount Model Approval and Hospital Preparations

In the United States, health system leaders discussed their preparations for the 340B Discount Model Pilot Program approved by HRSA (Health Resources and Services Administration) at a forum organized by Craneware Group. The eight manufacturer plans approved by HRSA on October 30, 2025 include Bristol Myers Squibb, AstraZeneca, Merck, Boehringer Ingelheim, Novo Nordisk, Pharmacyclics, Immunex, and Janssen. This program will officially start on January 1, 2026, and the doors of the existing 60-day preparation period are now open.

Lidia Rodriguez-Hupp, Craneware Group's Customer Relations Manager, stated, “With HRSA's announcement, the preparation process has begun.” She added, “During the next two months, hospitals, third-party administrators (TPAs), and technology partners must complete their testing, validation, and configuration during this time.”

Craneware Group showcased the 340B discount functionality on its cloud-based data platform, Trisus®, and summarized the main operational themes approved under HRSA's pilot framework:

Discount Priority: 340B discounts take precedence over Medicare Fair Pricing (MFP) discounts when both are applicable simultaneously.

Continuity: 95% of 340B processes will remain unchanged; the pilot program will apply to specific product codes (NDC) with high expenditures.

Payment Timing: Expected metrics are 10 days for 340B after reimbursement claims or MTF release, and 14 days for MFP.

Integration: Claims for pilot NDCs will flow through Beacon; Craneware Group will provide the necessary workflows, reconciliation, and transparency directly within the Trisus® platform.

Customers of Craneware Group confirmed that they can participate in the 340B Discount Pilot Program without requiring additional software or new vendor contracts. The discount workflow is ready as of today.

Lidia Rodriguez-Hupp emphasized, “Our customers should not have to track compliance and 340B benefits through new vendors or disconnected systems. Trisus presents the discount workflow in the trusted environment that hospitals already rely on for charge capture, compliance, and financial analysis.”

HRSA's approval signifies the transition from policy to operational application. All approved manufacturers will have 340B wholesale accounts over the WAC price, and covered entities will request discounts through Beacon after distributing sufficient medications to 340B patients.

The 60-day preparation period offers the last opportunity between October 30 and January 1 for the industry to configure their systems, test file arrangements, and validate compliance.
